MEDICAL DOCUMENT/PRE-PARTICIPATION FORMS

Trinity Athletes:

If you are planning to participate on a Trinity Christian College athletic team, one of the requirements before any participation is allowed is medical clearance. Medical clearance is monitored by the Trinity Christian College Athletic Training staff. Trinity Christian College Athletic Training utilizes a secure online electronic medical records system called ATS.

Please utilize this document as a means of reference for completing your medical documents for participation in athletics at Trinity for the upcoming school year. You will need to log in, create your account (for first time user), and complete all your medical information on the forms provided in the system.Ā Please follow the instructions and checklist below for your classification:

NOTE: Any student-athlete that will be under the age of 18 on August 1st will need a parent or guardian to assist with completion of the forms.


TIPS

  • Lack of planning and preparation on your part prior to your scheduled physical exam will not lead to an expedited response from Athletic Training staff. Please plan accordingly and complete the required ATS online requirements in sufficient time prior to your doctor's appointments. This process may take one to two hours depending on your proficiency to follow instructions.
  • Please read all instructions carefully.Ā Failure to comply with all instructions will delay your participation.
  • All student-athletes are required to have health insurance to participate in athletics. You will need yourĀ insurance cardĀ available to complete the 'Insurance' tab.
  • TheĀ physical formĀ that you will need to take to your physician's office for your required pre-participation physical examination is in ATS under the 'eFile' tab.Ā You will also need to complete the appropriate Medical History Form to bring to your exam appointment as well. Both forms will need to be signed, dated, and stamped by a physician.
  • All pre-participation physical exams must be performed in the United States by a licensed health care provider. An acceptable provider (MD, DO, NP, RN, PA, etc) varies by state.
  • You will need a medical exemption for anabolic agents or peptide hormones. This needs to be submitted and approved PRIOR to competition.
